What to bring with you to the emergency room or your doctor's appointments:
*Your insurance card and driver's license (ID)
*A list of your medications, allergies and surgeries. (If you do not have a medication list ready, please bring the bottles with you.)
*Your primary care physician's name and office phone number.

Kit Carson County Health Service District (KCCHSD) is committed to the prevention of disease and the promotion of health and healing. KCCHSD recognizes that tobacco and tobacco products are a health and safety hazard and therefore are not permitted inside our buildings.
All forms of tobacco used by employees, volunteers, medical staff, patients and visitors are prohibited inside all of our facility properties.
