Parke Health Medical Center and Stratton Medical Clinic are Rural Health Clinics and are committed to providing quality healthcare to infants, children and adults of all ages. Our goal is to help each of our patients to achieve their full potential as a healthy well-adjusted child or adult.

Our rural clinics, are proud to boast the services of two Physicians; Dr. James Perez, Dr. Zack Pimentel, three Family Nurse Practitioners; Mr. Jesse Smith, Ms. Susan Dorman and Ms. Michelle Wright.

Rural health care providers are often the first point of contact for patients, taking continuous responsibility for providing patient care and acting as advocates for the patient within the entire healthcare system. In a hometown clinic, the provider/patient relationship is friendly, allowing the providers to personally and professionally assess individual patients' medical needs.

At Parke Health Medical Center & Stratton Medical Clinic, we take great pride in our community members giving us the opportunity to serve them, and we look forward to many years of continued service. Parke Health Medical Center is located in the medical office building adjacent to the hospital upstairs and the Stratton Medical Clinic is located in a stand-alone brick building in Stratton, CO. Its address is 500 Nebraska Avenue, Stratton, Colorado.

Same-day appointments are generally available though they appreciate the appointment that is called in first. An appointment can be made for both facilities by calling 719-346-9481.

Our three physicians specialize in family practice as well as does our physician assistant. Both clinics share 3 Family Nurse Practitioners as well and they also specialize in family practice.

Parke Health Center and Stratton Medical Clinics are members of Reach Out and Read Colorado. Reach Out and Read promotes early literacy and school readiness in exam rooms. During well-child visits children ages 0-5 receive a book and we have gently used books for FREE in the waiting rooms.
