The Health Information Management department (HIM) maintains medical records as a support service of Kit Carson County Memorial Hospital.

Copies of medical records may be picked up at HIM. You may call 719-346-4770 to make an appointment to pick up medical records. Anyone requesting medical records must first complete the attached form listed below. In addition, please allow 72 hours advance notice and bring a picture ID.

Charges for copies are as follows for areas other than for continuation of care.

Pages 0-5 (No Charge)
Pages 5-15 ($10 Fee)
Pages 15+ ($10 Fee plus $0.25 per page)
